Medication Administration Requirements 
 
Dear Parent/Guardian,
 
If your child takes medication at school, please go to the Weston Public Schools website and download the necessary forms for the physician and parents/guardians to complete for the current school year.
 
On the WPS website, go to Quick Links, All Forms, and print the forms you need, or select from the links below:
 
For medication administration at school you will need:  
 
For asthma medications you will need:
 
For Epinephrine Autoinjector (EpiPen, EpiPen Jr., or Auvi-Q) you will need:
 
You may also stop by the nurse’s office at your child’s school to pick up the necessary forms.
 
Completed forms should be returned via US mail, email, or fax to:

A reminder:  Please pick up your child’s medication in the nurse’s office before the last day of school. Any medication left beyond the last day of school will be disposed of.

The nurse is available during school hours for direct care services, case management and confidential consultation.  Please call 781-786-5407 with any questions, concerns or changes in your child's health status.

 

Should you decide to keep your child at home from school due to any illness, please inform me by calling the Safe Arrival Line at (781) 786-5299 or call me directly.
